Apparently a fair copy of the index made in 1794 by John Caley, F.S.A., Keeper of the Records there (now Add. MS. 21325). Another similar fair copy, made circ. 1834, is Add. MS. 21326. Paper; ff. ii + 129. Folio. XIX cent. (watermark 1800). Bears the inscription 'J. Caley Hosp. Greien. 1802', f. 1. Evans's sale-cat., 27 July 1834, lot 2250. Subsequently belonged to Sir Thomas Phillipps, Bart. (MS. 6815, f. i; Sotheby's sale-cat., 24 June 1935, lot 119). B. Halliday's Cat. No. 193, 1935, item 209. Bookplate of the donor, f. ii. Presented by S.J. Madge, Esq., D.Sc., F.S.A.
