ff. 125r-141r: Matthew, Metropolites of Myra (circa 1550-1625), Κανὼν εἰς τὴν ἀχειροποίητον τῆς ὑπεραγίας θεοτόκου. An acrostich Ματθαῖος ὑμνῶ τὴν ἀπείρανδρον κόρην, written in the monastery of Kosinitza, inc. Μητέρα σὲ ἔγνωμεν ὑψίστου λόγου.

Ruling: 16 lines.
Decoration: Decorated headpiece (f. 16r) is inscribed 'χειρος Λαυρεντιος ἱεροδιακωνος' (sic) in a hand which is probably the same as that of ff. 1r-95v.
Binding: 18th century binding of blind-stamped leather over bevelled wooden boards with traces of two leather clasps. Stamping barely visible.
Script: Written by three hands: (1) ff. 1r-95v; (2) ff. 98r-141r; (3) ff. 144r-158v.

Owned by the priest Meletios of the Monastery of Saint Panteleimon (Roussiko), Mount Athos (ετουτως ὁ νομωκάνονας ὑπάρχοι του παπα Μελετηου ἀπο τω μοναστήρι τω ρούσικων); the priest Cyril, of the Monastery of Kosinitza, who bought it 'from the Holy Mountain'; ex-libris of Cyril and prayer dated 25 January 1748 in the hand of Βασιλικὸς του Μιχάλη απο το Κοβίκια. '116' in pencil (f. i).
