The authorship has been ascribed to Ḥammai Gaon.

Contains part 4, 3 and a fragments of part 2 of Vital's treatise. The heading states the the manuscripts was copied from the author's handwriting.

Appended is a piece called Yedi῾at ha-῾arugot, followed by a note on the angles of the 12 planets.
