Incipit: זאת העתקתי מן הפנקס דחבורתא קדישא קברנים דק"ק מנהיי' יצ"ו ה' יסור מאתנו ומכל ב"י כל חולה ומחלה. ואם ח"ו יקרה לא עלינו חולה אח' לשים אל לבו שאין אדם יודע עתו וחייו תלוים לו מנגדו שאם ח"ו פתאום נדחף מביתו ע"כ כתבתי איך יתנהג החולה בעודו שהוא בשכלו דהיינו קודם שהגיע עתו השטן עומד על ימינו לשטנו ואומר לו כפור באלהי ישראל בחלקלקות לשונו. ע"כ יעשה מסירה מודעה זו לפני עדים כשרים וינצל. ומה טוב אם יהיו עשרה אנשים אצל דבר זה. ואפילו בריא יתנהג לעתים יודעים בפרט בין כסא לעשור. נעשה במנהיי' תצ"ב לפ"ק ע"י סופר שמחה פיהם סג"ל. f. 1rContains some German text in Hebrew characters.
