Addressee is on f. 2v: ... שווערין אל הבתולה סרולע פאביל זוחמאן [גע]פונדין אצל בגאון הרב' אב"ד ק"ק ראגאזין.

A single leaf that is currently kept into a binding in between modern blank leaves serving as fillers. It is folded into two to form a bifolium.
