The groom's name is Kalonymus ben Menasheh Ḥefets (קולונימוס בן כמ"ר מנשה חפץ), and the bride’s name is Malkah daughter of Gavriʾel Pincherle (מלכה בת כמ"ר גבריאל פינקירלי).
Signed by the groom and two witnesses: Joseph Zechariah ben Nathan (יוסף זכריה בר נתן) and Meir ben Shimshon מאיר בר שמשון.
