Front flyleaf (ii)r: פנקס שייך להקטן יהודה ליב בן לא"א כהר"ר שמשון יצ"ו... נכתב יום ג'ד' אב תעב לפ"ק כאן ק"ק המבורג - התהלת כתיבתו יום ג' ב' חשון' ת'בא עלינו ג'אולה לפ"ק.
At the end of each entry, the scribe mentions his source: most often he refers to his teacher Rabbi Moses (שמעתי ממורי כהר"ר משה; e.g. ff. 2r, 11r, 19v, 20r, 29v).
He also mentions Abraham Joshua Heschel on f. 1r: שמעתי ממורי מהור"ר טוביה בשם הגאון הגדול מורינו ורבינו מהורר העשיל זצ"ל נשמתו ינוח בגן עדן אב"ד ור"מ בק"ק קראקא ; and Rabbi Yehudah: שמעתי ממורי מהור"ר יהודה (e.g. ff. 5v, 7v).
In some cases, the scribe is the author himself: מפי עצמי לפי ענית דעתי (e.g. ff. 4v, 5r, 7r, 17r, 20r, 26r).
