Alphabetically arranged handbook on pharmacology by Abū ‘Alī Yahyá ibn ‘Īsá (أبو علي يحيى ابن عيسى, d. 1100), known as Ibn Jazlah (ابن جزلة), composed for the Abbasid Caliph al-Muqtadī bi-Amr Allāh (المقتدي بأمر الله, reg. 1075-94).
The copy is defective at end; the final folio contains the beginning of Chapter lām (باب اللام, f. 269v).

الحمد لله الذي ظهرت بدائع مصنوعاته وبهرت غرائب مبتدعاته
ودل بظواهر صنعته على لطيف حكمته وأنعم على الخلائق بضروب نعمه وعم البرايا
بأنواع النبات والثمار المختلفة المنافع والبحار يسقى بماء واحد ونفضّل
بعضها على بعض في الأكل ...
