Alphabetically arranged handbook on pharmacology by Abū ‘Alī Yahyá ibn ‘Īsá (أبو علي يحيى بن عيسى), known as Ibn Jazlah (ابن جزلة; d. 493/1100), composed for the Abbasid caliph al-Muqtadī bi-Amr Allāh (المقتدي بأمر الله; reg. 1075-94, see f. 1v, line 11).
The manuscript was collected by the Scottish surgeon and naturalist Patrick Russell (1726-1805), probably during his service as physician to the Levant Company factory in Aleppo (1753-71).

ودل بظواهر صنعته على لطيف حكمته وأنعم على الخلائق بضروب نعمه

Condition: Edge margin mutilated and repaired without loss of text (ff. 1-2 and 121-128); minor tears and damage to paper throughout especially along ruling lines, some repaired (e.g., ff. 28, 29,93, 94, 118, 135-140, 200 and 209); final (twenty-second) quire lost and replaced (ff. 210-220)
Marginalia: Few by more than one hand (but see, e.g., ff. 50v, 125v, 131v and 162v), some mutilated or lost entirely through page trimming

- Muḥammad (محمد): his inscription recording acquisition of the manuscript at Istanbul and seal without legible (f. 1r);
- Abd al-Raḥmān ibn al-Shaykh Muḥammad al-Ḥakam (عبد الرحمان ابن الشيخ محمد الحكم): his inscription with date 1175/1761-2 (f. 1r);
- Ḥasan ibn al-Shaykh Muḥammad al-Ḥakam (حسن ابن الشيخ محمد الحكم): his inscription with date 1171/1757-8 (f. 1r);
- Tēr Serōp‘ Karmrts‘i (Կարմրցի Տէր Սերօփս): his inscription recording a visit to doctor Girgor (հէքիմ Գիրգորին, probably an error for Grigor) in Trapizon (Trebizond, now Trabzon in modern Turkey) for which he offers a prayer of thanks, dated 1201/1751-2 (f. 221v).
