The allocation of chapters is chapters 1-7 in vol. 1, chapters 8-13 in vol. 2, chapters 14-19 in vol. 3, chapter 20 formed the basis of vol. 4 but was considerably expanded in the course of publication. The foliation is that given in the manuscript.
