Contents:
ff. 1r-20r: The Order for the Visitation of the Sick; with the rite for Extreme Unction; imperfect at the beginning.
ff. 20r-49v: The Recommendation of a Departing Soul (‘commendacio omni fidelium defunctorum’); with neumes on red bars.
ff. 50r-89v: The Office of the Dead; with music notation (neumes) on red bars; imperfect at the beginning and end.
The manuscript contains a few later additions:
ff. 30r-30v, 47v-49r: texts added to replace missing leaves; written in the (?) 16th century.
Decoration:
Large and small initials in red ink throughout the manuscript; calligraphically decorated initials in black ink at the end; 2 inhabited initials in black ink, featuring an anthropomorphic head highlighted in red ink (ff. 67v, 88v).

Foliation: ff. 89 (+ 3 unfoliated paper flyleaves and 1 unfoliated parchment flyleaf at the beginning + 4 at the end); 1 unfoliated parchment stub between f. 14 and f. 15; f. 35 and f. 36; and f. 48 and f. 49; each quire has been mounted onto a paper guard.

‘Thomas Clarke’, owned in the 15th or 16th century: his name inscribed on f. 15v: ‘By me thom[as] clarke’ (not in Wright, Fontes Harleiani (1972)).
‘Thomas Stra[nton ?]’, owned in the 16th century: his name inscribed on f. 23r: ‘yours to youse [sic: use] in all duti Tho. Stra.’ (not in Wright, Fontes Harleiani (1972)).
‘Kemp’, owned in the (?) 16th or 17th century: inscribed f. 89v: ‘My sis[te]r Dennys Kemp oweth us [? xts xrs i rd], as appeareth by for her’ (not in Wright, Fontes Harleiani (1972)).
The Harley Collection, formed by Robert Harley (b. 1661, d. 1724), 1st earl of Oxford and Mortimer, politician, and Edward Harley (b. 1689, d. 1741), 2nd earl of Oxford and Mortimer, book collector and patron of the arts.
Edward Harley bequeathed the library to his widow, Henrietta Cavendish, née Holles (b. 1694, d. 1755) during her lifetime and thereafter to their daughter, Margaret Cavendish Bentinck (b. 1715, d. 1785), duchess of Portland; the manuscripts were sold by the Countess and the Duchess in 1753 to the nation for £10,000 (a fraction of their contemporary value) under the Act of Parliament that also established the British Museum; the Harley manuscripts form one of the foundation collections of the British Library.
