Foliation: ff. i + 48 (+ 3 unfoliated paper flyleaves at the beginning + 2 at the end); f. 16 is a paper fragment pasted onto a modern paper leaf each leaf has been mounted onto a paper guard and has a white paper strip attached to its outer margin.
Script: 17th-century script.
Binding: British Museum in-house. According to the Catalogue of the Harleian Manuscripts (1808), III, p. 303, the manuscript was bound together with Harley MSS 5178 and 5179 (Visitations of Shropshire) in the Harleian Library.

Provenance:
‘John Johnson’, perhaps the arms painter of that name who was Master of the Painter-Stainers Company in 1696, and from whom Humfrey Wanley purchased a number of his heraldic manuscripts for the Harleian Library, copied ff. 46v-48r in 1699: according to a note signed by him and with the date 1699 on f. 48r. If this 'Johnson' is the Master of the Painter-Stainers Company, then ff. 42r-45v may be item 57 ('Church Notes taken in Shropshire by William Smith Rouge-Dragon etc.') of the heraldic manuscripts in Quarto that are listed on the fragmentary catalogue of his manuscripts and books made by Wanley, which is preserved in Add MS 70488, Loan 29/261, envelope 5 (not listed under his entry in Wright, Fontes Harleiani (1972), p. 206).
The Harley Collection, formed by Robert Harley (b. 1661, d. 1724), 1st earl of Oxford and Mortimer, politician, and Edward Harley (b. 1689, d. 1741), 2nd earl of Oxford and Mortimer, book collector and patron of the arts.
Edward Harley bequeathed the library to his widow, Henrietta Cavendish, née Holles (b. 1694, d. 1755) during her lifetime and thereafter to their daughter, Margaret Cavendish Bentinck (b. 1715, d. 1785), duchess of Portland; the manuscripts were sold by the Countess and the Duchess in 1753 to the nation for £10,000 (a fraction of their contemporary value) under the Act of Parliament that also established the British Museum; the Harley manuscripts form one of the foundation collections of the British Library.
