Census declaration addressed to Cephalon, royal scribe of the Memphite nome, the declarant being Apollonius, son of Harphaesis alias Hephaestion and Thermouthis, aged 37. Apollonius registers himself and his five-year old child, Soterichus, whom he had from his ex-wife. He lives in rented property belonging to Sarapion, son of Sarapion. With imperial oath made by the declarant and subscription of the strategus Apollonius. With endorsement on the back.

Papyrus sheet, slightly torn at the top, lacking the bottom, with few small wormholes located in the lower half; written on both sides, along the fibres on the front and across them on the back. The front bears 45 lines, and the back has a two-line docket. Upper and left margins are preserved; no margin was left to the right. The papyrus is housed in a glass case.

Script: Small cursive hand; the first two lines are more cursive and compressed; the subscription at the end displays larger letters.
