Alphabetical handbook on pharmacology by Abū ‘Alī Yahyá ibn ‘Īsá (أبو علي يحيى ابن عيسى ), known as Ibn Jazlah (ابن جزلة; d. 493/1100). The full title is Minhāj al-bayān fī mā yast‘amaluhu al-insān (منهاج البيان فيما يستعمله الإنسان), but the title page (f. 1r) records the title as Fuṣūl al-sitta (فصول الستة) which is probably a reference to the six sections (fuṣūl/فصول) of the first chapter (maqālah/مقالة) of the text.
The text is divided into two maqālahs: maqālah I. ff. 21v-32r; ff. 32v-33r blank; maqālah II. 33v-62r.
Maqālah I begins (f. 21v, line 2-5):
وبعد حمد الله والثناء عليه بما هو أهله والصلوة على
نبيه المصطفى محمد وآله فإن هذا الكتاب هو ثاني
الكتب التي صنفناها في الطب التي الأول منها هو في
الأحكام الكلية من الطب والثاني منها هو هذا
الكتاب المجموع في الأدوية وقسمنا هذا الكتاب مقالين
Maqālah I ends (f. 32r, lines 1-4):
من الحيوانات الميتة بأمراض يحدث لها فهذه
هي القوانين الكلية التي يجب أن يكون عتيدة عند
الطبيب في أمر الأدوية المفردة والآن فإنا
نأخذ في المقالة الثانية ...
Maqālah I colophon (f. 32r, lines 4-9):
... وختمنا بالمقالة الأولى
في هذه المختصر بعون الله تعالى
وحسن تيسيره وصلى الله على محمد
خير الورى وعلى آله نجوم الهدى
وقد فرغنا من تعليقه يوم الأربعاء الثاث من رمضان
سنة تسعين وستمائة الهجرية والحمد لله حق حمده
Maqālah II begins (f. 33v, lines 2-6):
الحمد لله الذي ظهرت بدائع مصنوعاته بهرت
غرائب مبتدعاته ودل بظواهر صنعته على
لطيف حكمته وانعم على الخلائق بضروب نعمه وعم
البرايا بأنواع النبت والثمار المختلفة المنافع و
البحار يسقى بماء واحد ونفضل بعضها على بعض في
الأكل تتبارك الله أحسن الخالقين ...
Maqālah II end (ff. 61v, line 21-62r, line ):
كانت بعض الأسباب توجب القلة وبعضها توجب
الكثرة كانت ما يلقى من الدواء في المركب فقدارًا
معتدلًا ومن علم الأدوية المفردة يعلم قوة
[f. 62r] الدواء وضعفه ومنافعه وغير ذلك من أحواله
والله أعلم بالصواب وإليه المرجع
والمأب وصلى الله على خير خلقه
محمد وآله وصحبه وعترته
