The medical formulary of Amīn al-Dawlah Abū al-Ḥasan Hibat Allāh ibn Ṣā‘id (أمين الدولة أبو الحسن هبة الله بن صاعد) known as Ibn al-Tilmīdh (ابن التلميذ, d. 560 AH/AD 1165), chief physician at the ‘Aḍudī Hospital in Baghdad, and later court physician to the Abbasid Caliph al-Muqtafī (المقتفي, reg. 1136-1160). This copy was completed some 63 years after the author’s death, in 625 AH/AD 1227-28 by ‘Alī ibn Abī Ṭālib ibn ‘Alī ibn ‘Alī ibn ‘Alī ibn al-Ḥusayn al-Ḥalabī (علي بن أبي طالب بن علي بن علي بن علي بن الحسين الحلبي), probably at Baghdad (see title page f. 2v, lines 6-9, transcribed below).
The text is divided into 20 chapters (أبواب; see table of contents on ff. 2v-3r):

The lower margin of f. 128v and all of f. 129r contain notes and prescriptions in a hand other than that of the main scribe, ‘Alī ibn Abī Ṭālib al-Ḥalabī.

Decoration: Title page (f. 2a), title and author information in cloud bands in box with gold borders, on background of blue with foliate arabesque design partially shaded in red; below and to the left of this is a similar, but less ornate, box containing information relating to the scribe
Binding: Ottoman (?) binding in red leather with blind-tolled borders, medallion and pendants; no envelope flap
Condition: light waterstains, some foxing, and a notable amount bleed-through of black ink
Marginalia: Frequent marginalia by more than one hand
