On the lower half in the middle, a reference to the standard magical names (סנוי וכו'), followed by transpositions of מכשפה לא תחיה, etc. On the left, with a design in the shape of a hand, בן פרת יוסף וכו'. On the right, diagrams of the Star of David, inscribed with Shaddai, and other names.
On the four margins, the text והסיר יהוה ממך כך חלי וכו', etc.
