The lections of the Torah are written twice in parallel columns, once with the vowels and accents and once without. Cf Add MS 22413.

Musaf starts on folio 55r (also the same as in Or. 42).

Quires are numbered with Latin letters starting at the end of the codex with ‘a’.

The first leaf of the first quire is missing.

Purchase note, partially illegible:Yisra᾿el ben Daniyel (?) of Lisbon bought the book for 25 reals in 1384 (folio 179v).
Ya῾aḳov Tsoref: inscribed with his name on the margin: ‘I, Ya῾aḳov Tsoref, bought this mahzor for a significant amount of money, [5]454 AM (1694 CE) (folio 86v).
Remains of an erased inscription in Latin characters on front flyleaf (i)v.

Samuel Butler (b. 1774, d. 1839), headmaster and bishop of Coventry and Lichfield: his sale catalogue, Payne & Foss, 1841, no. 550, purchased by the British Museum: inscribed (folio [180v]).
