Treatise on the causes of the growth, lack and diseases of hair by Qusṭā ibn Lūqā (قسطا بن لوقا; ca 820-912) written for the vizier Abū Muḥammad al-Ḥasan ibn Makhlad ibn al-Jarrāḥ (أبو محمد الحسن بن مخلد بن الجراح, d. 882).
The treatise is divided into twelve chapters (باب), which are listed on ff. 186v-187r. The text ends abruptly near the beginning of Chapter Twelve.

كتاب قسطا بن لوقا اليوناني في علل الشعر ألفه لأبي محمد الحسن بن مخلد التماسي
أعزك الله تأكيد أسبابي عندك والقرب إليك يحملي على اتحافك بكل ما أجد السبيل
إليه وأعلمت أيدك الله أنك سألت عن علل الصلع والشيب عند الكبر ...
