Two quires from a fragment of a commentary on Ibn Sīnā's (ابن سينا, d. 1037) Canon of Medicine (القانون في الطب), defective at beginning and end. The present fragment comprises quires three and four of the original volume (ff. 2r-26v; see quire signatures on ff. 2r and 17r, and mid-quire marks on ff. 13r and 22r). An unknown number of folios are missing between f. 1 and f. 2.
The anonymous author says he was at Damascus in Shaʿbān 604/February-March 1208, when he came across twenty-five quires of notes (حواشي) on Ibn Sīnā's Canon of Medicine by the Jewish Egyptian physician al-Muwaffiq Shams al-Riyāsah Abū al-ʿAshāʾir Hibat Allāh ibn Zayn (الموفق شمس الرياسة أبو العشائر هبة الله بن زين, d. 1198) known as Ibn Jumayʿ (ابن جميع, see f. 1r, lines 13-15).
The only section heading in the surviving fragment of this manuscript is:
On the Third Book of the Book of the Canon (على الكتاب الثالث من كتاب القانون, f. 13r).

إن يسجد له وكان بمنزلة الفرزدق لما سمع القائل ينشد شعر
وجلا السيول عن الطلول كأنها زبر تجد متونها أقلامها فسجد فقيل له أتسجد للشعر
فقال أنا أعلم بسجدات الشعر وأنتم أعلم بسجدات القرآن ...
Ends (f. 26v, lines 17-19, defective at end):
... قال الرئيس في فساد الشهوة كمون كرماني ونامخواه لمضغان على
الريق وبعد الطعام ويؤكل سفوفًا صوابه يؤخذ أقول كلاهما جيد صواب قوله الطبن الجيد
