- Declaration addressed to the nomarch from Ophelas, son of Nemesion and Tereus, and his wife Hermione, and from his brother Castor, registering the birth of their son, Philantinoos alias Isidorus, twenty-five days old. The declarants refer to the benefits granted by Hadrian for children of Antinoopolis registered within thirty days of their birth. With names of three witnesses of the marriage and parentage, and subscription of Ophelas.

- Fragmentary papyrus sheet with two detached fragments, medium-dark brown; written on one side along the fibres, the back being blank. Portions of 21 lines remain; the upper margin survives. The detached fragment at the bottom bears a few letters from 5 lines and the lower margin. The papyrus is mounted on cardboard in a glass frame.
