- Lysimachus alias Didymus, son of Heraclides, notifies the nomarch of Antinoopolis of the birth of his son, Heraclides alias Valerius, aged 20 days, born to him by his wife Ninnarous. Lysimachus provides the names of three guarantors of his marriage and parentage; with oath of Didymus. The child is eligible for the privileges granted by Hadrian to children of Antinoopolis registered within thirty days from their birth.

- Papyrus, medium-brown, broken off on the left and lacking the lower left portion; with surface damage and a few insect-holes in places; written on one side, the back being blank. On the front, there are 26 lines lacking the beginnings; line 27 is in lacuna. Upper and lower margins survive; no space was left on the right. Vertical folds are still visible. The papyrus is mounted on cardboard in a glass frame.

Materials: Papyrus.
Dimensions: 246 x 228 mm; housed in a glass frame measuring 290 x 280 mm.
Script: Five hands. Neat and regular upright hand for the body of the document, with lines inclining upwards; second hand in lines 9-10, 13-17, thin and rapid cursive; third hand in lines 17-19, untidy cursive; fourth hand in lines 19-22, rapid cursive; fifth hand from line 23 to the end, unskilled and clumsy.

Provenance: Tebtynis, meris of Polemon, Arsinoite nome, Egypt (?). Purchased as part of a lot comprising Papyri 1873-2016 from Ali Abd el Hari on 15 May 1911.
