Paper, ink, typographic printing, manuscript. On the front side is a form of card filled with blue ink. It contains information about the right of treatment in all medical institutions of the region. On the back is a list of family members.
Extent: 1 sheet.
Size and dimensions of original material: 11,7 х 17,3.
Condition of original material: bend breaks; loss of a leaf fragment at the top (3 x 4 cm); numerous bend marks; surface contamination, greasy spots in the bottom center and right.
