- Dimensions: 220 x 150 mm.
- Binding: Housed in a separate volume.
- Decoration: Stained gold background with arches and columns filled with brightly coloured abstract ornamentation. Below each of the four extant arches, there is a medallion with a bust portrait (imago clipeata) painted in classical style,, three of which have halos, probably representing the Apostles.

Origin: Eastern Mediterranean (Constantinople?)
Provenance: Acquired at the Monastery of Simonopetra in Mount Athos by Antonios Trifillis and later by Anthony Askew (1722-1774). Purchased at Askew's sale, 15 March 1785, lot 622.
